Clinical Evaluations

IB/M Clinic and Student Teaching Evaluations

The clinic and student teaching experiences are pivotal components of Neag School of Education’s Teacher Education Program. Evaluations will be completed by the clinic teachers (or University Supervisors during the senior spring/student teaching semester) at both the midterm and end of the semester (finals). Links will be sent via online survey software, Qualtrics, to the teacher on record, and we ask that the evaluations are completed together with the Neag student(s).

PLEASE NOTE: The forms provided below are models only and are not to be used for actual evaluation. Evaluators will receive an email link to complete an electronic form for each of their students.

2022-23 Academic School Year

Junior Year (IB/M)

The clinic teacher completes the evaluation forms WITH their Neag student(s). Within one week after the due date, a PDF of the completed form will be distributed to respective student, clinic teacher, seminar leader, faculty advisor, and the Office of Assessment.

Senior Year (IB/M)

Senior Clinic Evaluation – Fall Semester Only

The clinic teacher completes the evaluation forms WITH their Neag student(s) during the fall semester. Within one week after the due date, a PDF of the completed form will be distributed to respective student, clinic teacher, seminar leader, faculty advisor, and the Office of Assessment.

Student Teaching (IB/M senior year spring semester)

Senior Student Teaching Evaluations – Spring Semester Only:

Student teachers and Cooperating Teachers will use a hardcopy of the midterm and final evaluations to complete a formal progress review prior to a 3-way meeting with the University Supervisor. It is the responsibility of the student teacher and Cooperating Teacher to complete the hardcopy evaluation before the University Supervisor arrives to review scores for the evaluation to be submitted. The scores on the evaluation form should represent a consensus between the Cooperating Teacher and the student teacher.

At the meetings for the midterm and final evaluations, the Cooperating Teacher and the student teacher will walk the University Supervisor through the evaluation form noting the student teacher’s strengths and areas of growth. The University Supervisor will also note the strengths and weaknesses they have observed, make additional comments on the form, and negotiate any disagreements in scores between the Cooperating Teacher and the student teacher. The University Supervisor will complete and submit the on-line evaluation form via Qualtrics based on that consensus.  Within one week after the due date, a PDF of the completed form will be distributed to the respective student, cooperating teacher, university supervisor, faculty advisor, and the Office of Assessment.

Internship Year (IB/M)

Internship Evaluation

The school internship supervisor completes the evaluation form with the Neag student(s). Within one week after the due date, a PDF of the completed form will be distributed to the respective student, school supervisor, seminar leader, faculty advisor, and the Office of Assessment.
